This Membership Agreement sets out the commercial terms of your Novus MD membership. It works together with the Terms of Service and Privacy Policy, which govern your use of the platform generally.
By enrolling, you selected one of the following tiers:

In addition to your monthly membership fee, a one-time onboarding fee applies when you enroll. It is billed once, at the time of your first payment, and does not recur.
What it covers: your initial comprehensive lab panel, your first physician evaluation, and the clinical and administrative work required to build your personalized care plan before your ongoing membership begins. It reflects the actual upfront cost of getting you properly evaluated — it is not an added markup on top of your membership.
The onboarding fee is disclosed in full, alongside your monthly price and commitment term, before you enter payment information at checkout — not after. It is billed once, per tier, at the amount shown in the table above.
The onboarding fee is non-refundable except as described in Section 7 (Refunds) or as required by law.
Your membership fee covers the diagnostics, physician access, and coordination described in your tier above. It does not include:
Membership is billed on a six (6) month commitment, paid monthly.
Your membership fee is billed monthly beginning on the date of your first payment. At the end of your six (6) month commitment, your membership continues on a month-to-month basis at the then-current rate until you cancel. We will notify you before any price change takes effect.
Each tier includes a set number of physician visits as described in Section 1. Visits beyond your tier's included threshold are billed at $150 per visit.
Your membership runs for an initial six (6) month commitment, as described in Section 3.
To cancel, email support@novusmd.com. We will confirm your request in writing.
After your six-month commitment. You may cancel at any time. Cancellation takes effect 30 days after we receive your request. You keep full access for the remainder of any period you have already paid for, and you are not billed again after that.
During your six-month commitment. Your membership is not cancellable as of right during the commitment period. If your circumstances change, email us and we will review your request — we do not hold people in a membership that is no longer right for them. Where we agree to end a membership early, the same 30 days’ notice applies, you keep access for the remainder of the period you have already paid for, and any fees already paid are handled under Section 7.
The onboarding fee is non-refundable once onboarding has begun, except as set out below or as required by law.
Membership fees are not refunded for a period that has already been billed. You keep access for the remainder of that period.
Where a refund is due, we refund what you have paid less our actual costs, which are your first month's membership fee and the onboarding fee.

Medications, supplements and other items billed separately are not covered by this section.

Enrolling in a membership does not guarantee a prescription or any specific clinical outcome. See the Prescription Policy, issued by Pelerin Medical Services, LLC, for details on prescribing standards.
We will provide notice before any change to your membership fee or tier structure takes effect. Continued use of the Platform after a change takes effect constitutes acceptance.
